Election Observers may use this form to report in at the end of their day, or if they have an incident that requires follow-up. Members of the public who witness election activity that is of concern may also submit a report.
Please provide details including the location (e.g. Name/Address of Vote Center, Drop Box, County Elections Office) where you observed the election activity. We like to hear good news as well as your concerns!
